When you first arrive, we will have a short form for you to complete. The New Chiropractic Patient Forms are available to download (below) and print. Completing these forms prior to your visit will save you time in our office. They are very simple and will help give your chiropractor the information he or she needs to better serve you. You will then have an initial consultation with your doctor to discuss your health concerns and goals and to determine the best plan for you!
Please bring your insurance card(s) with you (if applicable). As a courtesy to you, we will call to find out what your benefits may cover. We encourage you to call your insurance company ahead of time to verify coverage. Every policy is different, so we cannot guarantee how they will cover your care in our office.
