When you first arrive, we will have some short forms for you to complete.  They are very simple and will help give the doctor the information he or she needs to better serve you.  You will then have an initial consultation with your doctor to discuss your health concerns and goals and to determine the best plan for you!

Please bring your insurance card with you (if applicable) and as a courtesy we will call and find out what your benefits may be.

Stucky Chiropractic Centers also offers the required new patient forms online so you can complete them in the convenience of your own home, and save time in our office!
